Job Description
Title: Licensed Practical Nurse - Long-Term Care

Schedule: Must be available all shifts, including weekends and holidays as needed.

Assignment Details:
- Contract Length: 13 weeks
- Guaranteed Hours: 40 hours per week

Requirements:
- Active LPN license in the state
- Current BLS certification
- Minimum of one year of recent Long-Term Care or skilled nursing experience
- Experience with electronic medical records

Roles & Responsibilities:
- Provide compassionate, high-quality nursing care to residents in the Long-Term Care unit.
- Administer medications and perform assessments.
- Develop care plans and maintain accurate documentation.
- Coordinate care with physicians and the interdisciplinary care team.
- Communicate effectively with residents and their families.

Compliance / Notes:
- Holiday pay is based on hours worked on the holiday.
