Job Summary This is an excellent opportunity for recent graduates and other behavioral health professionals who are working towards independent licensure and are interested in the field of Integrated Behavioral Health. The Pediatric Intensive Case Manager/Behavioral Health Associate (PICM/BHA) will work alongside our medical staff and licensed behavioral health professionals to provide integrated behavioral health services to patients in the context of a medical visit. The PICM/BHA will work primarily with pediatric patients and their families to provide comprehensive, family-centered case management for pediatric patients with complex medical, behavioral, developmental, and psychosocial needs. Be part of a multidisciplinary team that's finding innovative ways to reduce barriers and bring quality behavioral healthcare to North Philadelphia! This position is grant-funded for an anticipated three-year period, pending final grant approval. Continued employment beyond the grant period is contingent upon funding availability and organizational needs. Note that this position requires proficiency in both English and Spanish. For otherwise qualified candidates who are not proficient in Spanish, we can provide up to 12 weeks of language immersion training in a Spanish-speaking country. What You Will Be Doing

- Receive and manage referrals for pediatric patients with complex care needs, collaborating with pediatric providers, behavioral health clinicians, and other departments to facilitate timely screening, intake, and care coordination.
- Provide ongoing intensive case management services to children and their families using a culturally sensitive, strengths-based, trauma-informed, and family-centered approach to build and sustain therapeutic relationships.
- Provide behavioral health assessments, consultations, and therapeutic interventions to patients and their families.
- Identify, coordinate, and facilitate access to appropriate services within Esperanza Health Center and the broader community, including specialty medical care, behavioral health services, Early Intervention, autism evaluations, school-based supports, and community resources.
- Support families in navigating complex systems, including healthcare, education, early childhood services, social services, and public benefits, to promote continuity of care and child well-being.
- Monitor and evaluate patient progress toward care plan goals, communicating updates, barriers, and concerns to the care team and adjusting plans as clinically and developmentally appropriate.
- Provide crisis intervention, safety planning, and coordination of urgent services for children and families experiencing acute psychosocial stressors, behavioral crises, or safety concerns.
- Maintain accurate, timely, and compliant documentation of services, care plans, and patient progress in the Electronic Medical Record (EMR).
- Dependent upon level of training and agency need, the PICM/BHA may also be tasked with seeing patients over the age of 18 to provide assessment, consultation, and brief interventions as part of integrative behavioral healthcare in the context of the patient's medical visit.

- Have a Master's degree in Social Work, Counseling, Psychology, Marriage & Family Therapy, or a related field; candidates with a Bachelor's degree actively pursuing a Master's may be considered
- Are pursuing or are eligible to pursue licensure as an LCSW, LPC, LMFT, or equivalent (candidates who are already independently licensed will be eligible for additional compensation).
- Are bilingual in English and Spanish (or willing to participate in immersive language training)
- Have experience working with children, adolescents, and families, particularly those with complex medical, developmental, behavioral, or psychosocial needs
- Demonstrate knowledge of trauma-informed care, child development, and family systems
- Are able to work effectively in a fast-paced, team-based, integrated care environment
- Are passionate about innovative approaches to improving pediatric outcomes and reducing barriers to care
- Desire to work in a Christian healthcare ministry committed to compassion, dignity, and holistic healing
- Ability to sit, stand, walk, and climb stairs
- Ability to lift up to 10 pounds
- Ability to work extensively at a computer and read fine print on a screen
