Are you passionate about helping patients navigate the complex world of healthcare coverage? We are looking for a dedicated and empathetic Patient Insurance Advocate to serve as a vital liaison between our patients, clinical staff, billing teams, and executive leadership.

In this role, you will empower patients with long-term, chronic health conditions to understand their coverage options, overcome financial barriers, and secure the care they need. If you have a background in medical insurance counseling and a heart for patient advocacy, we want you on our team!

Compensation: $21.00 - $23.00 per hour

Schedule: Monday - Friday, 8:00 AM - 5:00 PM (MST)

Location: Remote (Can sit in any state, but must work MST hours)

Overtime: Occasionally available/needed

Patient Support & Insurance Counseling

• Empower Patients: Assist Medicare patients in understanding their Chronic Special Needs Plan (C-SNP), Medicare, and Medicare Advantage options so they can secure their preferred coverage.

• Resolve Barriers: Investigate, address, or escalate complex coverage barriers and insurance concerns to ensure prompt, effective resolutions.

• Provide High-Touch Service: Deliver exceptional customer service, identifying changing financial needs and answering questions for patients managing chronic conditions.

Cross-Functional Collaboration

• Build Relationships: Act as the primary liaison connecting patients, facility teammates, central billing, and corporate offices.

• Partner with Clinical Teams: Work closely with Social Workers and other clinic staff to review patient lists, identify potential coverage issues proactively, and integrate insurance navigation strategies into daily workflows.

Reporting & Operations

• Track & Analyze Data: Manage internal tracking tools, evaluate trends alongside operational leadership, and prepare monthly analytical summaries on patient advocacy metrics.

• Portfolio Management: Compile monthly operational reviews on your assigned patient portfolio to ensure educational outreach and operational performance targets are met.

Mentorship & Team Support

• Foster Growth: Support team onboarding and professional development through peer mentoring and shadowing new hires.

• Contribute to Initiatives: Participate in special projects, attend team meetings, and help drive Insurance Management Team initiatives forward.

Required Skills & Qualifications

• High School Diploma.

• Demonstrable experience navigating Medicare programs, Medicare Advantage plans, and Chronic Special Needs Plans (C-SNP).

• Strong background in patient advocacy, medical insurance counseling, or healthcare financial coordination in a clinical or outpatient setting.

• Exceptional customer service and communication skills tailored to patients navigating long-term health challenges.

• Proven ability to track, analyze, and present operational data and portfolio metrics to leadership.

Preferred Skills (A Plus!)

• Professional background in specialized chronic care management (e.g., Nephrology or ESRD care).

• Experience collaborating directly with clinical Social Workers and multidisciplinary care teams.

• A history of mentoring, training, or onboarding colleagues on department procedures and best practices.
