Youth Health Advocate & School Outreach Specialist

The Italian Home for Children is seeking a Community Health Worker to promote health and wellness through school and local housing authority partnerships. You will act as a liaison, provide education and advocacy, and support youth with navigating the healthcare system and related services.

The role emphasizes outreach, data collection, collaboration with providers and community organizations, and on-site presence at least three days a week.
