Conducting Evaluations and Treatment Plans

• Conduct thorough evaluations for patients in various settings, creating tailored treatment plans to aid their recovery journey
• Implement physical therapy techniques aimed at enhancing mobility, strength, balance, and overall functionality
• Work closely alongside nurses, physicians, case managers, and fellow rehabilitation experts to ensure optimal patient outcomes
• Engage patients and their families by explaining therapeutic objectives, discharge plans, and home exercise regimens
• Maintain comprehensive documentation of evaluations, treatments, and patient progress as per regulatory and facility standards
• Participate in collaborative care conferences and team rounds to provide cohesive care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited program

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification is preferred
