Registered Nurse (RN) – Home Health

Full-Time | M–F, 8:00 AM–5:00 PM | Rotating On-Call

We are seeking an experienced Home Health RN to provide skilled nursing care, case management, and care coordination throughout Washington County, MD .

Position Details

40 hours/week guaranteed
M–F, 8 AM–5 PM
Rotating on-call: every 3rd weekend
30 points/week
Mileage reimbursement: $0.43/mile
Active MD RN or compact license required

Requirements

Home Care Home Base (HCHB) experience required
OASIS experience required
PICC line experience required
Wound VAC experience preferred

Responsibilities

Assess patients and develop, implement, and evaluate individualized care plans.
Provide skilled nursing care and support disease prevention and health maintenance.
Coordinate care with physicians and other healthcare providers.
Monitor medications and changes in patient condition.
Complete required clinical documentation and nursing notes.
Coordinate appointments, hospital transitions, and discharge planning.
Supervise LPN/LVN staff and support healthcare team members as needed.
Provide training and education related to patient health and safety.
