Registered Nurse - Home Health

Position Overview We are seeking a dedicated Registered Nurse to provide high-quality home health care to patients across multiple counties. This role focuses on delivering skilled nursing services, completing OASIS assessments, and ensuring comprehensive patient care in a home setting. The ideal candidate brings strong clinical skills, attention to detail, and an ability to work independently in a field-based environment. Schedule & Work Details Location: Coverage across Jefferson, Oldham, Henry, Trimble, and Carroll counties Shift: Monday through Friday, 8:00 AM to 4:30 PM Weekly Hours: 32-hour guarantee On-Call/Weekend Requirements: One weekend per month and one weekday per week Mileage Reimbursement: Yes, reimbursed between patient visits from first to last patient Pay Rate: $51 to $53 per hour Key Responsibilities Provide skilled nursing care to patients in a home health setting Complete accurate and timely OASIS assessments and documentation Monitor patient conditions and update care plans as needed Educate patients and families on disease management, medications, and care plans Collaborate with interdisciplinary care team members to ensure continuity of care Maintain compliance with agency policies, regulatory requirements, and best practices Manage a caseload efficiently while maintaining quality and patient satisfaction Required Qualifications Active Registered Nurse (RN) license Minimum of 1 year of nursing experience Ability to work independently in a field-based home health setting Strong clinical assessment and documentation skills Preferred Qualifications Homecare Homebase (HCHB) experience Prior home health nursing experience OASIS documentation experience Strong hands‑on patient care background and time management skills Benefits and Compensation Pride-Health offers eligible employees comprehensive healthcare coverage (medical, dental, and vision plans), supplemental coverage (accident insurance, critical illness insurance, and hospital indemnity), a 401(k) retirement savings plan, life and disability insurance, an employee assistance program, legal support, auto, home and pet insurance, and employee discounts with preferred vendors.
