Join our team as an embedded therapist within an integrated healthcare setting, collaborating with medical and behavioral health professionals to provide whole-person, coordinated care to individuals and families. JOB TITLE: Integrated Care Therapist LOCATION: Cape Fear Clinic – Wilmington, NC | On-Site SERVICES: Integrated Behavioral Health | Adult Population SCHEDULE: Monday–Friday, 8:00 AM–5:00 PM | Full-Time, 40 Hours/Week NO EVENINGS or WEEKENDS!!!

REQUIRED EDUCATIONAL/EXPERIENCE QUALIFICATIONS :

Graduation from a Master’s degree program in behavioral health Active licensure requirement LCSW preferred, or LCSWA, LCMHCA/LCMHC, or dual licensure with LCAS/LCASA, LPA

SUMMARY OF POSITION:

Provides comprehensive behavioral health services to individuals with co-occurring mental health and substance use disorders through individual, group, and family therapy. Conducts clinical assessments, develops evidence-based treatment plans, coordinates care across agency programs, and supports client engagement to improve treatment outcomes. Responsible for diagnostic assessments, case management, psychotherapeutic interventions, outcome tracking, and maintaining timely, accurate clinical documentation while adhering to professional licensure requirements and clinical standards.

POSITION HIGHLIGHTS:

Serve as an embeddedTherapist within an Integrated Care model. Provide crisis intervention services, as clinically appropriate, during regular business hours; no after-hours crisis response is required. Provide accessible behavioral health services in collaboration with medical and community providers. Primarily serve an adult population with complex mental health, substance use, and co-occurring needs. Provide individual, group, and family therapy. Conduct psychosocial and diagnostic assessments and develop individualized treatment plans. Utilize evidence-based and person-centered therapeutic approaches. Collaborate with primary care, behavioral health professionals, families, and community resources. Support warm handoffs, referrals, care coordination, and continuity of care. Provide crisis intervention and clinical support as appropriate. Maintain timely, accurate clinical documentation and comply with professional and regulatory standards. KNOWLEDGE & SKILLS: Knowledge of integrated behavioral health, clinical assessment, diagnosis, treatment planning, and evidence-based therapeutic interventions. Ability to collaborate effectively within an interdisciplinary care team. Strong clinical judgment, communication, and relationship-building skills. Ability to engage clients and families and provide person-centered, trauma-informed care. Ability to manage a complex caseload while maintaining accurate clinical documentation.
