Work Environment

This position is entirely onsite, as our therapists provide home care services to patients in their residences. Key Responsibilities

• Conduct thorough evaluations of patients in their home environments, assessing their physical capabilities, needs, and functional limitations.
• Create and implement customized treatment plans based on each patient's unique condition, employing methods such as targeted exercises, hands-on therapy, and the use of specialized equipment.
• Monitor patient progress, adapting treatment plans as necessary to ensure effective recovery.
• Train patients, their caregivers, and family members on appropriate exercises and mobility techniques to encourage independence and self-management.
• Document all treatment interventions and patient responses meticulously, maintaining accurate records to facilitate communication within the healthcare team.
• Stay abreast of developments in physical therapy practices through educational workshops, peer networking, and participation in professional organizations.

Patient Population / Client Focus

Qualifications

• A Doctorate in Physical Therapy.
