Responsibilities

• Evaluate residents’ functional abilities, including Activities of Daily Living, mobility, cognition, and psychosocial factors
• Develop and implement individualized occupational therapy treatment plans based on evaluations and physician orders
• Provide direct OT services in accordance with state and federal regulations and facility policies
• Monitor and document resident progress; adjust treatment plans as needed to meet goals
• Educate residents, families, and caregivers on strategies to promote safety and independence
• Supervise and provide direction to Certified Occupational Therapy Assistants (COTAs) and therapy aides as appropriate
• Ensure accurate, timely documentation in the electronic medical record
• Collaborate with the interdisciplinary team; participate in care planning meetings
• Maintain licensure and continuing education requirements as mandated by state regulations
• Support quality improvement initiatives and comply with CMS and facility guidelines

Requirements

• Graduate of an accredited Occupational Therapy program
• Current Occupational Therapist license in the state of practice
• Prior SNF or geriatric rehabilitation experience preferred
• Strong clinical reasoning, communication, and organizational skills
• Ability to work effectively in a team-oriented environment
