Role: Licensed Physical Therapist providing one-on-one care to older adults in private residences and senior living communities, utilizing the Geriatric House Calls™ model. Role: Licensed Physical Therapist providing one-on-one care to older adults in private residences and senior living communities, utilizing the Geriatric House Calls™ model. Responsibilities: Perform evaluations, develop individualized treatment plans, and deliver evidence-based therapy to support mobility and independence. Document patient progress accurately within the EMR system. Collaborate with an interdisciplinary team including PTs, OTs, SLPs, and Fitness Specialists. Educate patients, caregivers, and staff on safety and aging-in-place strategies. Incorporate telehealth and virtual care to extend access and flexibility. Qualifications & Skills: Valid Physical Therapy license or eligibility in the practice state. Degree from an accredited PT program. Basic computer and EMR proficiency. Current CPR certification. Ability to meet physical demands, including lifting, repositioning patients, and driving between locations.
