A skilled and compassionate physical therapist is needed to deliver home health services in a field-based setting. This contract assignment offers a rewarding opportunity to help patients enhance their mobility, strength, and independence within the comfort of their own homes.

Schedule and Assignment Details:

• 13-week contract assignment
• Monday through Friday, 8:00 AM 5:00 PM
• Caseload averages 2532 points weekly (typically 57 patient visits per day, depending on visit types)
• Daily travel to patient homes (mileage reimbursement provided)
• Comprehensive electronic documentation using Home Care Home Base (HCHB)
• Utilize OASIS assessments for required care episodes

Key Responsibilities:

• Complete thorough physical therapy evaluations, including OASIS assessments as needed
• Develop, adjust, and implement individualized plans to address mobility, gait, balance, pain, and safety
• Deliver targeted interventions such as therapeutic exercise, neuromuscular re-education, transfer training, gait training, and fall prevention strategies
• Monitor patient progress, update plans of care, and communicate changes with physicians and team members
• Educate patients and caregivers on exercises, safety protocols, use of assistive devices, and home exercise routines
• Ensure timely, accurate documentation in HCHB
• Collaborate within an interdisciplinary care team and participate in case conferences as required
• Comply with agency procedures, Medicare guidelines, and home health standards

Qualifications:

• Active Physical Therapist (PT) license in Washington (in good standing)
• Minimum of 1 year recent home health physical therapy experience
• Proficiency with Home Care Home Base (HCHB) preferred
• OASIS experience required
• Reliable transportation, valid drivers license, and current auto insurance
• Excellent time management, organization, and communication skills
