Premiums: Corrections Health Licensed Practical Nurses receive an additional 12% hourly premium and hourly differentials for working swing ($3/hr), graveyard ($6/hr), weekends ($3.50/hr) shifts. Mandatory Overtime*: The first 4 hours of the mandated shift will be paid at an overtime rate (1.5x), and the remaining hours of the mandated shift will be paid at a double-time rate (2.0x). In addition, if you are scheduled to work the following day after a mandated shift, then that schedule shift will be paid at an overtime rate (1.5x). Roles and Responsibilities Utilizing electronic reports and review provider schedules to proactively assess needs for prevention screening and communicating via EHR. Pro-actively performs pre-visit activities (e.g. scrubbing and huddling) for client visits as needed. As well as preparing patients for exams and procedures, including equipment set-up and specimen collection. Working with providers and teams to monitor chronic disease and prevention reports and initiates follow-up with clients or ensures appropriate referrals as needed. Following up on specialty referral reports, clinical treatments, prescriptions, medication cancellations, etc. Documenting clearly in the patient chart, using the SOAP method, if indicated, and EPIC "dot-phrases" and "smart phrases" as deemed appropriate per patient encounter. Supporting back office clinic flow by direct provider support as needed including rooming patients, ordering labs as directed. Providing after visit instructions based on nursing experience and understanding of client needs; makes follow-up appointments, if necessary. Administering medical treatments such as cleaning and dressing wounds, ear irrigations, suture removal, catheterizations, fluoride treatments, etc. Performs diagnostic tests such as labs, phlebotomy, EKG, etc. Work with RN and PCP to contribute to the plan of care. Completing documentation and charting as required by policy. Troubleshooting equipment issues and reporting necessary repairs as needed. Completing assigned Quality Control/Assurance activities. Minimum Qualifications / Transferable Skills Completion of an Oregon State Board of Nursing approved program at the certificate level or higher and a license to practice as a Licensed Practical Nurse as outlined by Oregon Revised Statutes (ORS) 678.021 A minimum one (1) year of nursing experience Valid CPR/BLS certification upon hire COVID-19 Vaccination Requirement: To protect the health of the community and employees, Multnomah County requires employees to be fully vaccinated against COVID-19 or have an approved medical or religious exemption as a qualification of employment. Preferred Qualifications / Transferable Skills* Ability to make observations concerning patients' general health conditions. Working knowledge of basic medication passing rules and protocols for patient safety, infection control and the limits of care governed by Medication Aide certifications. Ability to communicate clearly and effectively both orally and writing. Experience working in a Correctional Health Facility.
