Clinical Pharmacist Oncology

Registered Oncology PharmacistGraduate from an accredited school of Pharmacy with minimum of a BS in Pharmacy. NYS Pharmacist license and Current New York registration required. 2 – 3 years hospital pharmacy experience; Minimum three (3) of annual CE requirements must be in Chemotherapy/Oncology. Oncology Certification by NYSCHP or BPS or equivalent preferred. Experience in hematology oncology and infusion center operations preferred. Successful completion of Hospital and Departmental Orientation Program, according to hospital and Pharmacy policy and procedure.ResponsibilitiesPRIMARY RESPONSIBILITIESGathers necessary data to promote patient safety by ensuring that each relevant medication is indicated, effective, safe, and that the patient is adherent to therapyImproves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effectsAssesses relevant patient-specific medical problems, prioritizes medical problems based on urgency and severity, and identifies preventive and health maintenance issues as neededMay design a drug therapy plan for patient-specific problems and persuasively justifies recommendations
