Case Manager

Humana is seeking a Case Manager to support members across Green Lake, Marquette, Adams, and Waushara counties. In this role, you will coordinate integrated care for individuals with diverse medical, behavioral, and social needs. You will complete comprehensive assessments, develop and update person-centered care plans, and connect members with providers and community resources. Working in a member-focused, collaborative culture, you’ll educate members on benefits and treatment options, monitor outcomes, and ensure regulatory compliance.
