Lake Haven Nursing and Rehabilitation Center 1351 San Christopher Drive Dunedin, FL 34698 MDS RN Responsibilities: Coordinate the completion of Minimum Data Set (MDS) assessments for all residents in accordance with regulations Ensure accuracy and timeliness of MDS assessments and care plans Collaborate with interdisciplinary teams to develop individualized care plans Monitor and update resident care plans as needed Communicate effectively with staff, residents, and families regarding MDS assessments and care planning Stay up-to-date on regulations and guidelines related to MDS assessments Participate in quality improvement initiatives related to resident care MDS RN Qualifications Current RN license in the state of Florida - Minimum of 2 years of experience in MDS coordination in a long-term care facility Strong knowledge of MDS assessment process and regulations. Excellent communication and interpersonal skills, Ability to work effectively in a fast-paced environment. Detail-oriented with strong organizational skills.
