Hospice Registered Nurse (RN)

Schedule: Friday–Monday, 8:00 AM–6:00 PM

We're seeking a compassionate and experienced Hospice Registered Nurse (RN) to provide exceptional end-of-life care to patients and their families throughout York and Lancaster counties. This role is ideal for an RN who is passionate about delivering patient-centered hospice care in the home while working collaboratively with an interdisciplinary care team.

Position Details

• Schedule: Friday–Monday, 8:00 AM–6:00 PM
• Productivity Expectation: 6–8 visits per day
• Mileage Reimbursement: $0.62 per mile
• Territory: York and Lancaster Counties, Pennsylvania
• License Required: Active Pennsylvania Registered Nurse (RN) License

Responsibilities

• Complete hospice admissions, routine visits, PRN visits, and end-of-life (death) visits.
• Perform comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing care focused on symptom management, comfort, and quality of life.
• Collaborate with physicians, social workers, chaplains, aides, and the interdisciplinary hospice team to coordinate patient care.
• Educate patients and caregivers on disease progression, medications, symptom management, and end-of-life care.
• Monitor patient condition and communicate changes promptly to physicians and the care team.
• Coordinate medication management, ensuring safe administration and compliance with physician orders.
• Maintain accurate and timely clinical documentation in HomeCare HomeBase (HCHB).
• Supervise Licensed Practical Nurses (LPNs) and other clinical staff as appropriate.
• Participate in care planning, case management, and interdisciplinary team meetings.
• Ensure compliance with HIPAA, agency policies, and all applicable state and federal regulations.

Required Qualifications

• Active Pennsylvania Registered Nurse (RN) license.
• Hospice experience required.
• HomeCare HomeBase (HCHB) experience required.
• Current BLS/CPR certification.
• Valid driver's license, reliable vehicle, and current auto insurance.
• Strong clinical assessment, communication, and critical thinking skills.
• Ability to work independently while managing a home health territory.
