Advanced Practice Clinicians (APC) deliver clinical care, create shared care plans, provide education and resources within the Health Department and external programs that best meet the client's needs and health goals. An APC determines diagnosis and treats acute and chronic conditions, leveraging the resources and skills of an interdisciplinary health care team to improve or maintain health status for a diverse, multicultural client population within a busy, primary care setting. The Psychiatric Mental Health Nurse Practitioner provides professional, comprehensive specialized psychiatric nursing services and emphasis on prevention of disease and promotion of health.
