Job Summary The Pharmacy Clinical/Operations Specialist BMT is responsible for participating and leading pharmacy clinical or operational programs. Responsibilities include monitoring patient medication regimens, mentoring pharmacy staff, precepting students and residents, and evaluating and implementing clinical and operational initiatives. What You Will Do In This Role Interpret orders and transcribe or verify them accurately and efficiently into the pharmacy computer system Accurately compound and dispense pharmaceuticals Comply with facility policy and procedures and regulatory requirements Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose Provide clinical consultation and clarification to practitioners and suggest appropriate, cost-effective therapeutic alternatives to medical staff as needed Sustain the hospital formulary, minimizing nonformulary procurements and promoting rational drug therapy selection Attend and actively participate in daily multidisciplinary and bedside rounds Complete drug utilization reviews to promote rational drug therapy and implement improvements identified Provide accurate and timely medication information to healthcare staff and patients Lead and develop pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing and other programs as defined by Pharmacy Leadership; develop, manage and mentor the antimicrobial management program (AMP) Document all clinical activities and interventions accurately and completely; mentor and educate, and analyze interventions for accurate documentation and summary reports Support and participate in the organization's quality improvement, risk management, and patient safety program, including National Patient Safety Goals, Core Measures and HCAHPS medication-related initiatives Participate in design, collection, and reporting of adverse drug reactions, drug utilization reviews, medication errors and pharmacy quality initiatives Lead training and mentorship of pharmacists, pharmacy students, residents, technicians, nurses and physicians; develop staff competence and improvement plans when deficiencies are identified; lead preceptorship of pharmacy students and/or residents Maintain professional competency sufficient to meet current and future needs of the facility Provide and demonstrate expertise in a specialized area of pharmacy practice (operational and/or clinical) Provide basic technical support for automated dispensing machines and other pharmacy computer systems Effectively supervise pharmacy operation activities and provide support to the whole pharmacy team Comply with facility pharmacy schedule to work physically at patient care areas to interact with medical staff, nurses and other members of the healthcare team Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions Train and participate with Disaster Team drills, real-event responses and Code Teams as assigned; obtain and maintain BLS-ACLS or PALS current certification as appropriate Maintain appropriate controlled substance records; work with the Pharmacy Director and appropriate manager to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards; participate in controlled substance reviews and audits Comply with all regulatory standards; work with the Pharmacy Director to identify and develop processes to meet or exceed all regulatory standards; participate in quality and regulatory reviews and audits Prioritize work and lead others to comply with facility medication turn-around time policy and procedure; identify areas for improved efficiency and assist in implementation of process changes Supervise and assist pharmacy technicians and other support staff effectively Participate in establishing pharmacy goals and objectives and assist in implementation; participate with various nursing and medical staff committees as necessary Assist with staff performance evaluations; act as manager in charge as assigned Provide review and guidance on physician order sets and evidence-based medication use guidelines Assist with the preparation of the pharmacy newsletter and/or other forms of written and electronic communication Perform drug class reviews and drug monographs; participate in all aspects of the drug formulary management process Assist with development, implementation, and monitoring of pharmacy operations initiatives (operational/clinical); participate in the development and review of pharmacy policies and procedures Qualifications Education B.S. in Pharmacy from an ACPE-accredited School of Pharmacy OR Doctor of Pharmacy from an ACPE-accredited college of pharmacy Licenses/Certifications Current registered Pharmacist licensed by the TX State Board of Pharmacy Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy ACLS/PALS and NALS – required for critical care/ED position dependent on population served BCPS certification or other Board of Pharmacy Specialty (BPS) certification Experience Minimum of 3 years experience as an Advanced Clinical Pharmacist (in a specific area of focus) or at least 5 years of general hospital pharmacy experience, preferably within BMT or completion of a PGY2 residency or fellowship Employee completes initial and ongoing training and competencies as defined by service line, facility, and department/unit which are specific to the needs of the patient population served Equal Opportunity Employer We are an equal opportunity employer.
