Social Services Director

Address: 337 Neponset Ave Dorchester Full‑Time Social Services Director The Bostonian Nursing and Rehabilitation is seeking a Director of Social Services with up to a $7,500 sign‑on bonus to join and support our fantastic team caring for short‑term patients and long‑term residents. The facility is accessible by public transportation and has a private parking lot, located in the heart of Dorchester. Requirements Must possess a minimum of a B.A. in Social Work, LSW, or equivalent degree AND a current social work license. Masters in Social Work (MSW), LCSW, or LICSW highly preferred. 1+ year(s) skilled nursing facility experience, preferred. Job Duties Compiles social history data. Participates in assessments, development and evolution of resident's care plan. Responsible for planning and leadership of interdisciplinary care planning weekly meetings. Documents timely progress notes according to state and federal regulations. Coordinates the discharge planning for residents. Able to interpret and implement established social service policies, procedures and regulations. Coordinates services of community agencies for continued delivery of care upon discharge.
