Collaborate with clients, families, and healthcare teams to develop and implement goal-centered care plans, considering both quality and financial aspects of treatment. Provide hands-on skilled nursing care to clients of varying ages and acuity levels. Coordinate care with families, caregivers, and other healthcare providers. Medication Management: Ensure proper medication administration and reconciliation. Client Monitoring: Regularly monitor clients\' conditions and report any changes to the appropriate personnel. Provide education and support to clients, families, and caregivers. Ensure accurate and timely documentation of client care and progress. Ensure the safety needs of clients are addressed through home environment assessments and individualized abuse prevention plans. Provide clinical oversight, delegation, and leadership to other licensed and unlicensed personnel. Documentation: Maintain accurate and up-to-date client records. Compliance: Adhere to all relevant regulations, including OSHA and HIPAA. Manage your schedule effectively and participate in the on-call phone rotation on occasion. Qualifications Currently licensed and unrestricted as a Registered Nurse (RN) in the State of Minnesota and Wisconsin. Certified in CPR and have a negative TB skin test or chest x-ray. Minimum of one (1) years of experience in a health care facility or home care setting. Home Health Experience: Two or more years of clinical experience, with one year in a home health agency, primary care clinic, or health facility, is preferred. Driving: A valid driver\'s license and reliable transportation are necessary. Demonstrates critical thinking skills and the ability to make appropriate clinical judgments. Excellent written and oral communication skills. Proficiency in using computer systems and electronic charting is essential.
