This role is based onsite, in a home care setting, allowing you to work directly in the communities we serve. Key Responsibilities

As a Physical Therapist at Core & Agility, you will be responsible for:

  • Conducting comprehensive consultations with patients to evaluate their physical condition and symptoms.
  • Diagnosing movement dysfunction and developing individualized treatment plans based on evidence-based practices.
  • Instructing patients on proper therapeutic exercise techniques to aid their recovery.
  • Administering therapeutic modalities such as massage or stimulation to facilitate healing.
  • Assisting patients in the safe use of mobility equipment such as wheelchairs and walkers.
  • Maintaining meticulous patient records to document progress towards rehabilitation goals.
  • Providing education and guidance to patients and their families regarding in-home treatment options and therapeutic exercises.

Qualifications

The ideal candidate for this position will possess:

  • A Doctorate in Physical Therapy from an accredited institution.
  • A valid state license to practice physical therapy.
  • Strong communication skills, both verbal and written, to effectively interact with patients, families, and the healthcare team.
  • Proficiency in word processing and basic computer skills, essential for maintaining patient records and documentation.
  • A proactive, flexible attitude, and ability to adjust treatment plans as required based on patient feedback.

Preferred Experience

While a minimum of two years of relevant experience is preferred, we also welcome applications from new graduates who are keen to learn and grow within a supportive environment.
