Licensed Practical Nurse (LPN) - Home Care

Covering areas within zip codes 19104 and 19139.

This is a 13-week contract position.

Shift: 8:00 AM - 4:30 PM

We are seeking a qualified Licensed Practical Nurse (LPN) with at least 2 years of experience in home care. The ideal candidate must be proficient in wound care and possess strong clinical skills to provide high-quality patient care in a home setting.

Responsibilities:

• Administer medications and provide patient care as per care plans.
• Perform wound care and manage dressings effectively.
• Monitor and document patients' vital signs and health status.
• Communicate effectively with patients, families, and healthcare team members.

Required Experience / Certifications / Licensure:

• Must have a valid LPN license.
• 2+ years of experience in home care preferred.
• Proficiency in wound care is required.
