Clinical Nurse Lead

Job Summary Monday through Friday, 8-5 with hour lunch, no weekends, no on‑call, with paid holiday time off and paid CEU. The Clinical Nurse Lead is the market’s subject matter expert on Nursing, Medical Assistant, Lab and Radiology roles, responsibilities, and evidence base best practices. The Clinical Nurse Lead works in collaboration with the Practice Manager who directly supervises ancillary clinical staff, including Team Nurses, Treatment and Triage Nurses, Medical Assistants, Radiology Technicians, and Phlebotomists and who is also responsible for team member hiring, onboarding, staffing, and performance. The Practice Manager serves as the accountable leader for operational performance, staffing, and compliance, while the Clinical Nurse Lead serves as the clinical subject matter expert responsible for execution, training, and day‑to‑day clinical workflow support. This role collaborates with the Medical Director, Market Presidents and operations leads to provide oversight of clinical nursing care and operations, including primary care nursing, medical assisting, same‑day access, and immediate care. The Lead Nursing and MA Specialist is responsible for onboarding and training all new clinical ancillary staff. The Clinical Nurse Lead is responsible for local implementation of the point of care CLIA waived laboratory program, radiology program, and infection control program. As a local leader for a value‑based care organization, the Lead Specialist ensures the nursing department contributes to meeting all clinical, utilization management, and regulatory compliance goals. This role is also a practicing Team Nurse or Treatment and Triage Nurse. Duties and Responsibilities Oversight/Administrative: Supports orientation, onboarding, training, competency validation, and workflow adherence for clinical team members Guides new hires in their dynamic relationship with other clinical ancillary staff to cover overlapping responsibilities on team Collaborates with shared services nursing leadership to introduce education sessions and trainings that encompass both virtual and office capacities Act as subject matter expert for all nursing roles and responsibilities, including PCP panel management and primary care nursing and medical assisting activities, Patient Centered Medical Home (PCMH) standards, Immediate Care protocols, lab protocols and radiology protocols Serves as the first‑line point of contact for the nursing, medical assistant, lab and radiology questions and direct support needs Ensure timely and accurate completion of clinical tasks, workflows, and documentation in alignment with organizational standards and regulatory requirements Monitors and reinforces compliance with organizational policies, regulatory requirements, and patient care standards Provides day‑to‑day guidance, support, and oversight to nursing and medical assistant staff to promote accountability, teamwork, and service excellence Prioritizes clinical operations, manages competing demands, and supports efficient patient flow across the care setting Identifies workflow gaps and leads or supports process improvement initiatives to enhance efficiency and care delivery Coaches staff, addresses workflow issues, reinforces clinical and operational standards, and collaborates effectively with providers and operational leaders Models professional behavior, maintains patient confidentiality, exercises sound clinical judgment, and escalates issues appropriately Serves as a clinical resource, mentor, and role model for nursing and medical assistant staff Supports center leadership with coverage during planned/unplanned PTO, leaves of absence, and unanticipated schedule gaps Oversee management of medical supply and floor stock medication inventory Manage quality assurance and training for point of care CLIA waived laboratory Manage quality assurance and training for radiology policy and procedure Facilitate completion of all regular logs: eyewash, safety checklist, fridge/freezer, temperature monitoring, emergency checklist, quality control, etc Take the lead with medical emergency response Collaborate with operational center leadership on employee health, blood borne pathogens exposure, and temperature monitoring Proactively engage with the entire integrated care team and assist with resolving barriers to care and clinical services Attend/Chair weekly center meetings in collaboration with operational leadership Maintain excellent communication between integrated care teams and site and national leadership Other assigned duties from the center leadership team Clinical: Team Nurse: Act as a practice‑based nurse, supporting on‑site providers in facilitating high quality primary care that promotes appropriate healthcare utilization Supports providers during patient visits, including clinical assessments, medication administration, injections, and point‑of‑care testing Manages same‑day clinical needs, walk‑in patients, and triages acute complaints in person or by phone Responds to and assists with onsite medical emergencies, including assessment, oxygen administration, medication support, and coordination with EMS in collaboration with immediate care area if applicable Manages PCP schedules and optimizes daily clinic flow to support access and productivity Leads panel management activities by tracking patient needs, prioritizing care, and ensuring follow‑up on action plans Facilities visit orchestration by actively managing the day’s clinical flow, so members are seen, provider time is organized, and the schedule stays productive by dynamically adjusting visit format, sequencing and handoffs in real time Reviews and acts on provider care plans, ensuring all identified tasks and follow‑ups are completed Coordinates and tracks orders, including labs, imaging, DME, and referrals, ensuring timely completion and resolution of delays Manages inbound and outbound communication, including phone calls, patient messages, and coordination across care team members Provides medication reconciliation, patient education, and adherence support to optimize treatment outcomes Supports transitional care activities, including follow‑up after emergency department visits or hospitalizations Delivers chronic disease management in alignment with care pathways, standing orders, and provider direction Identifies and addresses quality care gaps through proactive review of patient panels and coordination with the care team Engages patients through coaching and education to support disease self‑management and improve outcomes Conducts outreach to patients lost to care and facilitates scheduling of follow‑up appointments Connects patients with internal and external resources, including social services and community‑based programs Collaborates with multidisciplinary team members, including providers, medical assistants, pharmacy, behavioral health, and care management Participates in daily huddles and contributes to prioritization of patient care needs and clinic operations Monitors emergency department alerts and initiates outreach and follow‑up for patients not engaged in care management Coordinate with the Immediate Care Area to provide in person and telephonic triage. Perform nursing interventions including wound care, IV (intravenous) therapy, and phlebotomy as needed Provides coverage for staffing gaps and supports medical assistant or triage functions as needed to maintain clinic flow including point of care testing, medication administration, and office flow Leverage strong time management skills to prioritize and balance member care responsibilities with provider panel management and role‑specific tasks Treatment and Triage Nurse: Performs telephone and in‑person triage of patients presenting with acute complaints, determining appropriate level of care and disposition in collaboration with providers Serves as the primary access point for walk‑in patients and same‑day clinical needs, ensuring timely evaluation and intervention Perform clinical nursing care, including, but not limited to telephone and in‑person triage, central and peripheral line care, infusions, wound care, medication and oxygen administration, inhalation therapy, and medical emergency management Conducts focused nursing assessments and delivers direct patient care, including monitoring, evaluation, and clinical interventions Provides advanced clinical treatments such as IV therapy, infusions, wound care, inhalation therapy, and medication administration Administers oxygen therapy and supports management of acute and chronic clinical conditions Responds to and manages medical emergencies within the clinic setting Collaborates with primary care providers and same‑day providers to determine patient care plans and appropriate next steps Documents triage encounters and clinical care activities accurately in the electronic health record, including telephone encounters and in‑person visits Participates in quality assurance activities for point‑of‑care and CLIA‑waived laboratory testing Ensures patient access to care by adhering to established clinical workflows, policies, and procedures Coordinates with the integrated care team to resolve barriers to care and facilitate timely services Provides patient education and coaching on disease management and appropriate use of healthcare services Communicates effectively with interdisciplinary team members using structured communication techniques to ensure safe and coordinated care Identifies patients at risk for worsening conditions and initiates escalation or higher level of care when appropriate Supports efforts to reduce unnecessary emergency department utilization by managing acute needs in the outpatient setting Assists with continuous improvement initiatives and problem solving to enhance access, quality, and patient experience Minimum Qualifications Bachelor’s degree in nursing Active, unrestricted license as a Registered Nurse in the state of practice required. Compact RN license preferred, or willingness to obtain Minimum of 3–5 years of clinical nursing experience required, preferably in ambulatory, primary care, urgent care, or outpatient settings Minimum of 1–2 years of experience in a lead, charge, preceptor, coordinator, or informal supervisory role required Experience supporting, guiding, or overseeing Medical Assistants, Lab and/or other clinical support staff required Demonstrated experience in clinical workflow coordination, staff support, patient flow management, and crossfunctional collaboration required Experience with quality improvement initiatives, clinical competencies, training, and policy/procedure implementation preferred Active CPR certification Experience working for a community‑based organization preferred Excellent interpersonal communication, problem solving, and organizational skills Independent self‑starter and critical thinker interested in population health, healthcare utilization management, and the ongoing development of our care model Proficiency with Microsoft Office applications and in electronic health records (EHR), clinical documentation standards, and basic computer applications required Working conditions This job operates in a professional office environment. This role routinely uses general office equipment. Often wear a special uniform, such as a lab coat or surgical scrubs, protective or safety attire, such as gloves and masks. Exposed to the diseases and infections of patients daily. May share workspace with other technicians and staff.
