Licensed Practical Nurse (LPN/LVN) – Home Health
Location: Grants Pass, Oregon
Sign-On Bonus: $10,000
Schedule: Full-time, Monday–Friday
Remote: No; field-based position

Responsibilities

• Provide skilled nursing visits in patients’ homes according to physician-prescribed plans of care

• Monitor patients’ conditions and communicate changes to the appropriate clinical team members

• Educate patients and caregivers on medications, treatments, and ongoing care needs

• Complete accurate, timely, and thorough clinical documentation

• Collaborate with physicians, nurses, therapists, and other members of the care team

• Travel throughout the assigned coverage area to support patients in their homes

Qualifications

• Graduate of an accredited practical or vocational nursing program

• Active Oregon LPN or LVN license

• At least one year of clinical nursing experience

• At least one year of home health experience

• Current CPR certification

• Valid driver’s license and ability to travel throughout the assigned territory

• Strong clinical judgment, communication, and documentation skills

Pay Details: $32.00 to $40.00 per hour
