Annandale Village is a nonprofit community supporting adults with developmental disabilities and acquired brain injuries. Our residents—known as Villagers—live with dignity, purpose, and connection across our campus.

Our Community Access Program creates opportunities for Villagers to build independence, strengthen life and vocational skills, develop meaningful relationships, and participate in the broader community. Through individualized experiences and community-based learning, Villagers are supported in pursuing their interests, working toward personal goals, and becoming active participants in the communities around them.

Position Purpose

The Community Access Program Professional provides person-centered support to Villagers as they build skills, pursue individual goals, explore interests, and participate meaningfully in their community.

This role implements Individual Service Plan (ISP) goals through community experiences, vocational activities, life-skills development, social opportunities, and other individualized programming. Through encouragement, instruction, and meaningful relationships, the Community Access Program Professional helps Villagers develop confidence, strengthen independence, and experience greater connection to the community around them.

Major Duties and Responsibilities

Supporting Individual Goals and Skill Development

• Implement individualized goals and supports identified within each Villager's Individual Service Plan (ISP). 
• Provide instruction, encouragement, and individualized support to help Villagers develop life, vocational, social, and community-based skills. 
• Create meaningful opportunities for Villagers to practice skills through everyday experiences and real-world environments. 
• Adapt activities and levels of support based on each Villager's abilities, preferences, communication style, and goals. 
• Encourage Villagers to participate as independently as possible while providing the support needed for success. 
• Observe Villager progress and provide meaningful feedback to support continued growth and person-centered planning. 

Creating Meaningful Community Experiences

• Support Villagers in identifying and participating in community experiences that reflect their interests, preferences, goals, and support needs. 
• Plan and implement activities that promote independence, inclusion, social connection, recreation, vocational development, and community participation. 
• Support Villagers in making choices, building relationships, developing confidence, and advocating for themselves within the community. 
• Safely transport and support Villagers during community activities, events, and other assigned experiences. 
• Represent Annandale Village professionally and positively while supporting Villagers in community settings. 

Supporting Villager Health, Safety, and Well-Being

• Maintain awareness of Villager health, safety, emotional well-being, and individualized support needs during program and community activities. 
• Follow established health, safety, behavioral, and support plans and provide positive behavioral support as appropriate. 
• Provide personal care and other individualized assistance as needed while protecting Villager dignity, privacy, and independence. 
• Observe and promptly communicate changes in Villager health, behavior, participation, or support needs. 
• Follow infection prevention, transportation, emergency response, and organizational safety procedures. 
• Provide CPR, First Aid, and emergency assistance within the scope of training when required. 

Documentation, Communication, and Teamwork

• Complete required documentation related to Villager participation, ISP goals, services, health, safety, and other assigned areas accurately and timely. 
• Track and communicate Villager progress toward individualized goals and provide feedback that supports ISP development and review. 
• Work collaboratively with Community Access employees and other members of the interdisciplinary team to provide consistent, coordinated support. 
• Communicate respectfully and professionally with Villagers, families, coworkers, community partners, and other stakeholders. 
• Participate in ISP meetings, required training, program activities, special events, and Saturday programming as assigned. 
• Help maintain program spaces, equipment, vehicles, and materials in a safe, organized, and welcoming condition. 
• Protect Villager dignity, rights, privacy, and confidentiality. 

Other Responsibilities

• Maintain required training, certifications, and competencies. 
• Follow Annandale Village policies, procedures, safety standards, and applicable regulatory requirements. 
• Support changing program and Villager needs as assigned. 
• Perform other duties in the best interest of Annandale Village and the Villagers we serve. 

Required Qualifications

• High school diploma or GED. 
• Ability to read, understand, and follow written and verbal instructions. 
• Ability to use internet-based programs and electronic systems required for documentation, training, and communication. 
• Ability to provide individualized support to Villagers with varying abilities and support needs. 

Preferred Qualifications

• Two years of experience working with individuals with developmental disabilities, acquired brain injuries, or other special populations desired. 

Employment Requirements

• Must be at least 18 years of age. 
• Valid Georgia Class C Driver's License, proof of insurance, and acceptable MVR. 
• Must meet Annandale Village requirements for operating organizational vehicles and transporting Villagers. 
• Must pass GBI and FBI background checks, including fingerprinting. 
• Must pass drug screening and tuberculosis screening. 
• Must complete and maintain required CPR, First Aid, and other assigned training and certifications. 

Physical Requirements

• Ability to actively participate with Villagers in community, vocational, recreational, and skill-building activities. 
• Ability to sit, stand, walk, bend, reach, climb, kneel, crouch, and perform other movements associated with providing direct support. 
• Ability to provide personal care and mobility assistance when needed. 
• Ability to lift up to 25 pounds independently and move up to 75 pounds with assistance. 
• Ability to safely assist Villagers into and out of vehicles and transport Villagers in the community. 
• Ability to perform CPR, First Aid, and emergency response duties as required.
