Job Summary

We are seeking a reliable, adaptable, and experienced Travel Licensed Practical Nurse (LPN) to join our clinical team in South Burlington, VT. In this assisted living setting, you will provide compassionate, high-quality care to residents with diverse health needs.

This contract role offers flexible shift scheduling across days, evenings, and nights, allowing you to maximize your routine while supporting resident health, safety, and independence.

Key Responsibilities

• Resident Assessment: Regularly assess residents' health status, monitor vital signs, and promptly report physical or behavioral changes to supervising RNs and physicians.

• Medication Administration: Safely administer prescribed medications and treatments in accordance with facility protocol, state scope of practice, and eMAR standards.

• Clinical Care: Perform basic clinical procedures, wound care, and dressing changes under supervision as assigned.

• Documentation: Maintain clear, precise, and timely clinical records in the facility’s Electronic Health Record (EHR) system.

• Care Coordination: Collaborate closely with RNs, physicians, CNAs, and multidisciplinary care teams to carry out and update individual resident care plans.

• Resident & Family Support: Educate residents and family members on health maintenance, wellness strategies, and safety measures.

Qualifications & Skills

Required

• Education: Graduate of an accredited Practical Nursing (LPN) program.

• Licensure: Active, unencumbered LPN license in the state of Vermont (or valid compact/multi-state license with eligibility for VT practice).

• Certifications:

  • Current CPR / BLS for Healthcare Providers certification.

  • First Aid certification (or equivalent).

• Experience: Prior nursing experience in an assisted living, long-term care, or geriatric care environment.

• EHR & MAR Skills: Proficiency in electronic health record systems and electronic medication administration records.

• Flexibility: Willingness and availability to work variable shifts (days, evenings, nights, and weekends as needed).

Preferred

• Background in managing chronic health conditions in senior living populations.

• Strong communication skills and demonstrated ability to thrive in a collaborative, team-oriented residential facility.
