Clinical Lead Manager- Central Community IV Therapy Team Clinical Assessment, planning, implementation and evaluation of all aspects of patient care, and developing treatment/care plans that may need to be delivered from a range of options. To develop new skills in IV therapy delivery, using innovative techniques and sharing these with the wider IV Therapy Team ensuring competencies are met. Following a holistic assessment, devise an individualised care programmed for each patient, modifying it as required Assess patients holistic needs, communicating highly complex and sensitive information to patients and careers as to their assessment, diagnosis, prognosis and treatment plan Be aware of Assistive Technology and utilise it in patient care where appropriate Where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, explanations require adjustments in order to gain understanding, consent and concordance Be required to use tact and persuasive skills in order to gain the patients cooperation in their treatment/care/management plan Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care coordinated appropriately To provide patient focused education, training and advice to patients, carers, other professionals and voluntary organisations. To participate in multi-disciplinary/multi-agency meetings as appropriate, e.g. Gold Standard Framework Plan and organise specialist services within the team Provide clinical expertise and advice to others, including Social Services, GPs and other agencies Ensure activity data and clinical information is recorded on SystmOne and completed on a daily basis May be exposed to bodily fluids, infected material, blood products, therefore must utilise universal precautions and adhere to infection control policies Ensure informed consent is obtained prior to initiating interventions Work with staff to ensure competencies are maintained To maintain clinical credibility by acting regularly as a clinical practitioner in own area of expertise Professional Leadership Be required to undertake risk assessments and act upon them appropriately Be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs Ensure own paperwork and records are maintained, and also those maintained by the team Monitoring equipment and supplies via the electronic ordering systems used within the team To work with and deputise for the Service Lead Engage and lead on triage and referral to discharge standard processes Actively contribute to integrated team meetings and other meetings where appropriate and chair meetings as necessary Develop and maintain effective working relationships and communication with all members of the organisation, other professionals and agencies Instigate, and actively participate with the Clinical Operational Managers in the development of the operational strategy and project management Contribute to working groups within the Locality and organisation Support all team members within the integrated team, respecting them, their roles and contributions Ensure effective use of resources, both financial and human, by members of the team Be aware of budget and ensure all effort is made to work within this Participate in the recruitment process, using the opportunity to skill mix to meet the demands of the service Attend training as required to develop current job role Oversee safe use and maintenance of equipment and supplies Remain accountable for own professional actions as determined by the professional bodies Participate in evidence-based practice, using research methodology Produce induction programmes for students and new starters Be accountable for work delegated to others within the team, ensuring staff are competent to undertake delegated work Ensure working environment is conducive for learning and an appropriate learning experience for students Provide mentorship for students undertaking pre/post registration courses Provide advice/training to team members Mentor existing staff and provide training in specialist area as appropriate Demonstrate leadership skills To relay information from the Management team in respect of NCH&C and local issues to all members of the team on a regular basis in the form of business meetings. To assist in the resolution of any conflicts, resolving informal complaints locally and assisting in the investigation of and response to formal complaints as required. To participate in the staff appraisal scheme as both an appraisee and appraiser, maintaining a dynamic personal development plan. Be aware of the Trusts behaviour framework and ensure behaviours are embedded in role and team and utilised when managing and supervising staff. To actively contribute to the modernisation and forward movement of NCH&C, being actively involved in and taking an interest in changes within the NHC, the potential impact of local and national policies and the implications for the service. To demonstrate a sound understanding of clinical governance and risk management, and apply to work situation, and ensure the team do likewise; identifying, assessing and documenting risk of implementing change To actively perform absence management, submitting forms in a timely manner, as per the Trusts Policy To be responsible for the compliance of Mandatory training and appraisals within the team, ensuring KPIs are achieved To be an authorised signatory for goods, services and payroll. To accurately complete personal human resources documentation in respect of team members. Ensure that designated time frames are adhered to. Complete all data returns as required by NCH&C. Collate report, manage and analyse information on activity levels and quality, in accordance with agreed performance standards To monitor the delegated team budget and assist the Locality Management Team to effectively manage resources To monitor and manage staff absence, mandatory training, annual leave and study leave in line with NCH&Cs policies, procedures and guidelines. Inform the Locality Manager of any significant trends/issues. To comply with existing organisational and departmental policies, procedures and guidelines and to be actively involved in the reviewing and updating of them as appropriate.
