Responsibilities include implementing treatment plans, monitoring patient progress, educating patients and caregivers, maintaining accurate documentation, and ensuring compliance with agency policies and regulatory requirements.Key ResponsibilitiesPatient Care & TreatmentProvide occupational therapy interventions as prescribed by the physician and outlined in the Occupational Therapist’s plan of care.Assist patients in improving functional abilities and achieving established treatment goals.Observe, evaluate, and report changes in patient condition to the supervising Occupational Therapist.Patient & Family EducationEducate patients and caregivers on disease processes, treatment plans, and home safety practices.Instruct patients and family members on the proper use of adaptive equipment and assistive devices, including wheelchairs, walkers, canes, braces, crutches, and prosthetic/orthotic equipment.Documentation & ComplianceComplete clinical documentation accurately and timely in accordance with agency policies, Medicare regulations, and federal guidelines.Maintain compliance with all applicable healthcare regulations and professional standards.Team CollaborationCommunicate effectively with physicians, therapists, nurses, patients, families, and other healthcare professionals.Attend required in-service trainings, staff meetings, and continuing education activities.Maintain communication regarding scheduling and availability with supervisors and scheduling staff.QualificationsRequiredCurrent Occupational Therapy Assistant (OTA) license in the state of employment.Minimum of one (1) year of OTA experience in an acute care setting.Valid driver's license and current automobile insurance.Ability to meet the physical demands of the position.PreferredPrevious home health or home care experience.Skills & CompetenciesCompassionate and patient-centered approach to careExcellent communication and interpersonal skillsStrong attention to detail and organizational abilitiesAbility to work independently while collaborating with a multidisciplinary teamDependable, professional, and adaptableCommitment to delivering high-quality patient careAbout UsAdena Home Health is proud to be part of the Alternate Solutions Health Network family.
