JOB SUMMARY

Under the supervision of the Director of Clinical Services of PACE, manages flow of the PACE of West Baltimore Medical Clinic. Coordinates services with the primary care team, responsible for the daily management of the activities of the Clinic, to ensure all processes are performed in a timely and efficient manner. Responsible for the hiring, training, competency testing, supervision and performance evaluations of the Medical Assistants, LPNs, and clinic RNs.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The principle duties of this position shall consist of, but are not limited to, the following:

1. Takes lead in effectively communicating with multi-disciplinary team and customers at meetings and throughout the day. 

1. Assists the Director of Clinical Services and QAPI/Intake Specialist in meeting all customer service standards and responding to participant and employee complaints.

1. Responsible for the hiring, training, supervision, performance evaluation and corrective action of subordinate staff for the following positions: Clinic RNs, LPNs, MAs

1. Engages in team building. Coaches and mentors staff. Instructs and teaches staff as needed. Trains staff to function independently. 

1. Sets clear expectations and assigns appropriate authority to staff; holds staff accountable for their scope of work and compliance to company policies and procedures.

1. Understands the motivational needs of staff, thereby recognizing and rewarding employees in an appropriate manner.

1. Evaluates the overall effectiveness of the Clinic.

1. Assists the Director of Clinical Services with development of written protocols for the Clinic.  Assists the Director of Clinical Services to determine need for protocols to be developed into specific policies and procedures for the medical clinic. 

1. Responsible for ensuring PACE of West Baltimore is compliant with State and Federal regulations and guidelines. 

1. Participates in the investigation, review and presentation of Level II reports and Root Cause Analysis to State and CMS Officials. 

1. Actively participates in the coordination and observations of annual facility drills. Participates in drill review and makes recommendations and effects policy changes. 

1. Coordinates the annual inspection of facility medical equipment and facilitates ongoing repairs. 

1. Applies expert knowledge to facilitate change as a result of market forces, research findings, and regulatory agencies.

1. Ensures timely and effective coordination of participant care with outside contracted service providers: nursing homes, assisted living, lab, oxygen, etc.

1. Reconciliation for invoices for pharmacy and clinic supplies. 

1. Reviews and audits clinic staff Electronic Health Record entries and charting for errors, record strikes, deletions, and compliance with documentation standards and policies.

1. Work with primary care staff to develop specific patient education programs.

1. Will participate in on-call coverage, as needed with input from the Director of Clinical Services.

1. Maintains fiscal accountability for clinic provided which includes, but is not limited to, oversight of team expenditures and payroll/overtime. Ensures that team members are scheduled in an appropriate manner to support productivity expectations and limits use of overtime via creative, proactive hiring.

1. Participates in the Quality Assurance program and imparts upon their departments the role of all staff in the QA.

1. Demonstrates active participation in strategic planning for clinic departments and the overall PACE program. 

1. Delivers patient care as appropriate with in the scope of advanced practice nursing.

1. Assists in the development and implementation of policies, procedures, and standards which affect the care of the participants.

1. Develops and maintains a collaborative working relationship with all physicians and the health care team members involved in the care of the participants.

1. Identifies and implements practices and procedures that are research based to enhance the quality and cost outcome of the participants.

1. Serves on various committees as requested.

1. Assists the Director of Clinical Services with the development of clinic supplies budget. Assists in the monitoring of expenditures to meet budget goals. Regularly reviews and signs off on staff mileage reimbursement requests. Biannually completes a full review of mileage reimbursement, with assistance from the Director of Clinical Services. Monitors expenditures with commitment to good stewardship of the organization’s resources.

1. Responsible for semi-annual renewal of CLIA waiver for clinic laboratory. 

1. Considers the impact of decisions on other departments and the company as a whole, while demonstrating effective partnership management skills.

1. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.

1. Follows all PACE of West Baltimore Policies and Procedures and OSHA safety guidelines.

1. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.

1. Maintains a courteous, helpful and professional attitude on the job. Displays a willingness and ability to be responsive in a warm and caring manner to all customer groups.

1. Consistently cooperates and supports organization in problem solving issues. Ensures customer satisfaction. Will support the success of the entire healthcare team. 

1. Maintains a safe working environment. Follows PACE of West Baltimore Safety Policies and Procedures.

1. Participates in continuing education classes and any required staff and training meetings. 

1. Additional responsibilities will be added at managers discretion, as career growth opportunities and responsibilities are constantly arising in our flexible and start-up environment.

• Experience crafting policies, procedures and other written processes.

• Ability to actively participate in strategic planning for both the Home Care Department and the overall PACE program.
• Ability to communicate clearly and effectively, both verbally and in written formats.
• Proficient in Excel and Microsoft Office programs, including Outlook. 
• Ability to work independently with minimum supervision. 
• Skilled in identifying and recommending problem resolution.
