RN Case Manager, Home Health

Role Summary A growing healthcare organization is seeking a Registered Nurse (RN) Leader to oversee clinical operations within its Home Health service line. This role is responsible for supervising field clinicians, ensuring high-quality patient care, and supporting clinical and operational excellence. The position serves as the primary liaison between field staff and leadership, driving coordination, compliance, and performance improvement initiatives.

- Serve as the primary point of contact for field clinicians, providing guidance, coaching, and mentorship
- Oversee clinical operations, including care delivery, documentation, and compliance with regulatory standards
- Manage and supervise a team of clinicians, ensuring high-quality, patient-centered care
- Review and approve physician orders, clinical documentation, and care plans
- Facilitate case conferences and interdisciplinary team (IDT) meetings
- Conduct and support utilization management reviews and clinical audits
- Ensure accurate and timely completion of OASIS assessments for quality reporting and reimbursement
- Monitor patient progress and adjust care plans to optimize outcomes
- Support quality improvement initiatives and ensure adherence to organizational policies and standards
- Coordinate scheduling and caseload management to maintain efficiency and continuity of care
- Participate in on-call rotations and respond to urgent patient care needs as required
- Active Registered Nurse (RN) license in the state of practice
- Minimum of 1 year of experience as a Registered Nurse
- Prior leadership experience in a Home Health setting
- Hands-on experience with OASIS documentation (HCHB experience preferred)
- Strong clinical, organizational, and communication skills
- Ability to lead, mentor, and support clinical teams
- Current RN license in the state of practice
- Active CPR certification
- Valid driver's license with insured, reliable transportation and ability to travel within the assigned territory
- Salary: $110,000 - $135,000 annually (based on experience)
- Schedule: Full-time, Monday-Friday (8:00 AM - 5:00 PM)
