This position provides coverage across multiple clinic locations throughout Indiana, including Columbus, Anderson, West Lafayette, Hammond, and Fort Wayne. This is an excellent opportunity for an experienced provider who enjoys practicing independently in a patient-focused primary care environment while supporting preventive health, wellness, and occupational healthcare initiatives. In addition to hourly compensation, mileage reimbursement is provided for travel between assigned clinic locations.Schedule: PRN/Float schedule based on clinic coverage needs. Travel required between assigned Indiana clinic locations. Mileage reimbursement provided.Key Responsibilities:Perform pre-employment physicals and annual wellness examinationsEvaluate, diagnose, and manage acute and chronic medical conditions using evidence-based clinical practicesDevelop and implement individualized treatment plans within supervising physician protocols, when applicableAssess and treat workplace injuries and common illnessesDeliver preventive care services, health screenings, and wellness programsEducate patients on healthy lifestyle choices and disease preventionSupport weight management through nutrition, exercise, behavioral coaching, and medication management, including GLP-1 therapies when appropriateOrder, review, and interpret diagnostic testsMonitor patient progress and adjust treatment plans as neededCoordinate referrals to specialists and other healthcare providersMaintain accurate and timely documentation in the electronic medical recordCollaborate with clinic staff to deliver high-quality patient careAssist with additional clinical and operational responsibilities as neededRequired Qualifications:Board Certified Nurse Practitioner or Physician Assistant with an active Indiana license (or ability to obtain)Minimum of 1 year of experience in Family Medicine, Internal Medicine, Urgent Care, or another primary care settingComfortable practicing independently in a clinic environmentStrong clinical assessment and decision-making abilitiesCurrent BLS CertificationExcellent interpersonal and patient communication skillsCommitment to preventive, patient-centered healthcareWillingness and ability to travel between assigned Indiana clinic locationsPreferred Qualifications:DEA RegistrationExperience in employer-sponsored health clinics, occupational medicine, or wellness programsExperience managing patients participating in weight management programsFamiliarity with obesity treatment medications, including GLP-1 therapiesExperience working with multidisciplinary healthcare teamsKey Skills Required:Primary Care ManagementPreventive & Wellness CareChronic Disease ManagementClinical Decision MakingOccupational HealthPatient Education & Health CoachingElectronic Medical Records (EMR)Adaptability Across Multiple Clinical SitesWhy This Location?
