Physical Therapist

Full Time Physical Therapist Case Manager Anticipated Territory: Grass Valley. Position Overview As a Physical Therapist Case Manager, you will provide skilled services in patients’ homes, evaluate and develop individualized care plans, coordinate care with physicians, supervise PTAs, and document patient progress. All positions require field visits to patients’ homes. Responsibilities Perform home evaluations and develop individualized care plans. Assist patients with mobility, movement, and regaining independence. Create detailed treatment and care plans for each patient. Communicate and coordinate with patients’ physicians to ensure quality care. Evaluate and report patients’ progress to MD. Supervise Physical Therapy Assistants (PTAs) as permitted. Document patient progress and maintain accurate medical records. Coordinate with interdisciplinary team members. Qualifications Current unrestricted CA Physical Therapist License. Current BLS/CPR Certification. Valid CA Driver’s License. Knowledge of local community resources. Understanding of individual development, human behavior, and cultural influences on health care. Knowledge of state and federal homecare regulatory guidelines (preferred). Experience in home health care (highly preferred). Experience with high acuity patients (a plus). Experience working with an interdisciplinary team (highly desired). Case Management fundamentals (desired). Self‑directed, empathetic, critical thinker, outstanding interpersonal, leadership, organized. Benefits & Compensation Mileage/Vehicle Reimbursement: $0.70 per mile between visits. Competitive compensation: $120,000 – $135,000 plus $10,000 annual bonus, paid quarterly. 401(k) with up to 4% match.
