Job Opportunity

Position Title: Physical Therapy Assistant (PTA)

Assignment Duration: 13 weeks

Schedule: Monday - Friday, 8:00 AM - 5:00 PM

Job Responsibilities:

• Provide physical therapy services to patients in their homes.
• Conduct start of care and follow-up visits as required.
• Document patient interactions and maintain accurate records within required timelines.
• Participate in periodic office meetings and communicate effectively with the care team.
• Maintain reliable transportation and adhere to agency-required insurance standards.

Qualifications:

• Minimum one year of experience in adult home health or hospice care.
• Current CPR and BLS certifications are required.
• Experience using HomeCare HomeBase and OASIS EMR systems.
• Must have a valid driver's license and auto insurance.
• Ability to travel up to 75 miles from the home base clinic.
• Comfortable with driving longer distances between patients.

Productivity Expectations:

• Maintain a productivity standard of 30 units per week, equating to 40 hours of work.
• Average patient load of 5-10 patients per day.
• Complete all required documentation within 24 hours of patient visits.

Travel and Coverage:

The position will cover patients in Ware, Pierce, and Brantley Counties. Mileage will be reimbursed at the IRS standard rate for travel between patient homes. The commute from the first patient to the clinic and back at the end of the day is not billable.

Additional Information:

• Flexibility in accepting patient assignments is expected. Declining assignments may result in cancellation of the contract.
• A commitment to on-call rotations may be required based on location and staffing needs.
