Admissions Coordinator / Social Worker

Position Overview Act’s Admissions Coordinator/Social Worker will provide clinically related social services to support the well‑being of residents and their families. The role involves participating in the admission process in collaboration with the Administrator and Director of Nursing, assessing residents’ psychosocial needs, and delivering individualized support services to enhance their quality of life. Participate in the admission process in collaboration with the Administrator and Director of Nursing. Assess residents’ psychosocial needs and deliver individualized support services to enhance quality of life. Requirements Bachelor's degree in Social Services. Master’s degree is preferred. Minimum of two years’ experience in a long‑term care setting. Must be either a Licensed Certified Social Worker (LCSW) or Licensed Certified Social Worker‑Clinical (LCSW‑C) when employed in Maryland.
