Job Summary:
We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients, providers, and other members of our team. At least 2 years of experience and exceptional skill is required.

Responsibilities:
- Perform patient assessments and administer medications.
- Develop and implement treatment plans.
- Provide skilled nursing care and case management.
- Collaborate with interdisciplinary healthcare teams.
- Deliver hospice care to individuals with serious illnesses.
- Maintain accurate patient records and documentation.
- Communicate effectively with patients, families, and healthcare providers.
- Active Registered Nurse license in the state of working.
- Bachelor's degree in Nursing or related field.
- Minimum of two years hospital, home health, or hospice care experience.
- Clinical assessment, medication administration, and treatment planning skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Driver's license, car insurance, and automobile for field work.
- Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
- Advanced certifications relevant to nursing specialties.
- Experience with interdisciplinary team collaboration.
- May require on-call availability depending on the work setting.
- Work environment may include hospice, ICU, ER, or other critical care areas.
- Work schedule may vary depending on facility needs; shifts can include days, nights, weekends, and holidays.
- Shift assignments based on staffing requirements and patient care needs.
