Requirements:
Active HI RN License
1+ year Medicare-certified Home Health experience
Homecare Home Base & OASIS required
Valid driver’s license & car insurance
COVID & Flu vaccines required

Details:
Rotating weekends (2–3/month)
On-call: 6 shifts/month
Mileage reimbursement provided
Case management experience preferred
