Case Manager RN
This is a 13-week contract position.
Shift: 4x10 or 5x8 Days, with start times varying between 07:00 AM and 08:30 AM.
We are seeking a dedicated Case Manager RN to join our team. The candidate should possess a current AK RN license and have experience in case management within an acute care setting. Responsibilities will include discharge planning, care coordination, and ensuring adherence to CMS guidelines and patient care standards.
Conduct discharge planning for patients.
Coordinate patient care and collaborate with interdisciplinary teams.
Ensure compliance with CMS and Joint Commission standards.
Utilize Epic for patient management and documentation.
Perform continued stay reviews and determine medical necessity.
Minimum of 1 year of acute care experience required.
BLS certification required, ACM certification preferred.
Active AK RN license is required for auto offer.
Epic experience is mandatory.
