Social Worker - LCSW at Providence Mission Hospital in Mission Viejo, CA. This position is Full Time and will work 8-hour, Day shifts.

The Palliative Care Social Worker works collaboratively on an interdisciplinary consulting team to support patients and families dealing with serious illness. The Palliative Care Social Worker is responsible for providing a full range of social work services including psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention, resource referral, short term counseling, grief support, advance care planning, facilitation of family meetings in collaboration with interdisciplinary team. The Palliative Care Social Worker assumes a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes. The Palliative Care Social Worker takes an active role in JC Certification in Palliative Care.

Required Qualifications:

• Master's Degree in Social Work (MSW or related field)
• Upon hire: California Clinical Social Worker License
• 2 years social services experience with a health care related organization.

Preferred Qualifications:

• 3 years experience in a multi-disciplinary health care setting and specialized training in palliative care and/or hospice.
