Family Medicine, Nurse Practitioner/Physician Assistant

Job Description Choice Medical Group has been serving the local area for over 30 years and has a network of over 200+ physicians. Choice Medical Group is physician owned and located in Apple Valley, CA. We are seeking a compassionate and detail-oriented Nurse Practitioner or Physician Assistant to join our care team. This role focuses on delivering high-quality preventive care through Annual Wellness Visits (AWVs), accurate HCC (Hierarchical Condition Category) recapture, and comprehensive medication reconciliation. Youll play a critical role in improving patient outcomes and supporting value-based care initiatives. you will have a full staff, full benefits, including Medical, Dental, Vision, Life, and a very generous 401k offering. QUALIFICATIONS: Must have valid, and kept current/in good standing throughout employment: CA Registered Nursing License CA Nurse Practitioner License CA Nurse Practitioner Furnishing License CA Physcian Assistant License DEA BLS ACLS Must not be on OIG/GSA exclusion list (qualified to see Medicare patients) Minimum 1 year of clinical experience (HCC or primary care experience preferred) Strong knowledge of Medicare Advantage, HCC coding, and risk adjustment Proficient in EMR documentation and medical coding best practices Excellent communication and patient engagement skills Preferred Skills CRC (Certified Risk Adjustment Coder) certification Bilingual (Spanish/English) Experience with in-home assessments or mobile care, willing to travel to local clinic sites within the Victorville, Apple Valley, Phelan, Hesperia. Key Responsibilities Conduct Annual Wellness Exams (AWVs) in accordance with CMS guidelines Identify and accurately document chronic conditions for HCC risk adjustment Perform thorough medication reconciliation and patient education Collaborate with primary care providers and care coordinators to ensure continuity of care Utilize EMR systems to document findings and support coding accuracy Participate in quality improvement initiatives and compliance audits As an Equal opportunity employer, DBA Choice Medical Group is committed to a diverse workforce.
