Palliative Care RN & Case Manager (Community)

VITAS Healthcare Corporation is seeking a Palliative Care Registered Nurse (RN) to support its growing program in the community. You will act as Case Manager, coordinating patient care, supporting discharge planning, and educating patients and families about disease processes and treatment options. The role requires a minimum of 2 years as an RN, 1–2 years in case management, and community/home health experience is preferred.
