License Practical Nurse (LPN)

Licensed Practical Nurse (LPN) The Lakes Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to work with Pediatric and/or adult patients in their own homes. Responsibilities: Utilize the nursing process to assess, plan, implement, and evaluate patient care. Provide skilled nursing care/services in accordance with prescribed orders on each assignment. Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition. Collect, analyze, and interpret data and information from healthcare members and documents actual and/or potential nursing diagnoses. Qualifications: Currently licensed as an LPN/LVN in the state in which the LPN will practice. Current TB or Chest X-Ray. Current BLS card. One year prior professional nursing experience preferred.
