Details Home health setting providing skilled visits to patients covered by Medicare, Medicare Advantage, and commercial insurance Diverse patient population including med/surg, cardiac, ortho, hips/knees/shoulders, post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, and amputee (AKA/BKA) patients Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM) 6-8 home visits per day with a minimum of 6 points per day 1-2 weekends required per contract Self-scheduled visits and hours based on office needs Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems Provided with laptop, MiFi for internet access, and portable printer Requirements Active FL or Compact OT license required (if applicable) Minimum 1 year of experience required; 2 years preferred Must be an independent practitioner BLS certification required OASIS training and LSVT certification preferred Home health experience required Valid driver's license and personal vehicle required for home visits Cerner charting experience preferred COVID vaccination or exemption documentation required Flu shot or declination required; mandatory mask wearing if declined Additional Information Provide occupational therapy services including ADLs, adaptive equipment training, home assessments, pre-discharge planning, and wheelchair evaluations Perform standardized assessments including cognitive assessment, FIM, and functional capacity evaluation Treat conditions including CVA, amputee, arthritis, dementia, hand injuries, multiple sclerosis, nerve injuries, Parkinson's disease, spinal cord injury, and traumatic brain injury Utilize modalities including electrical stimulation, myofascial release, lymphedema management, and burn/wound management Any solid color scrubs accepted as dress code Candidates must reside more than 100 miles from the facility to qualify for travel rate Former BayCare employees must wait at least 12 months before returning as a traveler RTO is not accommodated for standard travel contract submissions American Traveler Job ID #P-753169.
