Position: Licensed Practical Nurse (LPN)
Setting: Long-Term Care / Nursing Home
Shift: Days – 12-hour shifts
Schedule: Rotating shifts
Charting System: PointClickCare (PCC)
Contract Length: 13 weeks

Job Description

We are seeking experienced Licensed Practical Nurses (LPNs) for a day-shift position in a long-term care setting. This assignment offers a rotating schedule with 36 or 48 guaranteed hours per week, along with opportunities for additional overtime.

Responsibilities

• Provide direct nursing care to residents in a long-term care environment.
• Administer medications and treatments according to physician orders and facility policies.
• Monitor residents and report changes in condition to the appropriate healthcare team.
• Perform nursing assessments and maintain accurate resident records.
• Follow individualized care plans and nursing protocols.
• Collaborate with CNAs, RNs, physicians, and other healthcare professionals.
• Accurately document patient care using PointClickCare (PCC).
• Follow infection-control and safety procedures.
• Provide compassionate, resident-centered care.
• Maintain effective communication with residents, families, and members of the healthcare team.

Requirements

• Current and active LPN license required.
• Previous long-term care or nursing home experience preferred.
• Ability to work rotating 12-hour shifts.
• Must be flexible with the facility's assigned schedule.
• Strong clinical, communication, and organizational skills.
• Ability to work effectively in a team environment.
