Behavioral Health Whole Person Care Coordinator/ Registered Nurse (RN) - Hybrid in Central Virginia

Sentara Health Plans is hiring for a Behavioral Health Whole Person Care Coordinator/ Registered Nurse (RN) - Hybrid in Central Virginia City/State Richmond, VA Work Shift First (Days) Position Status: Full-time, permanent (40 hours/week) Position Location: This is a hybrid role based in Central Virginia (Richmond, Petersburg, Cumberland, Prince George, Hopewell, Brunswick, Franklin, Spotsylvania, Fredericksburg, Stafford, King George, VA and the surrounding areas). The role includes travel of up approximately 2 days per week in the community (home, CSB, or hospital visits as needed) and 3 days working from home. Standard Working Hours: 8 AM to 5 PM EST, M-F Overview This position will work with members under Sentara Health Plans who are both medically and behaviorally complex. The population will include children, adolescents, and adults. Education Bachelor’s Degree (Required) Certification/Licensure Required license must be valid in state of practice.
