What You'll Do

• Complete 69 in-home health assessments/day
• Review medical history and risk factors
• Collect vitals, labs, and screening data
• Document findings in EMR
• Educate members on next steps
• Coordinate follow-up if red flags show
• Travel across Multnomah County only

Must-Haves

• Active NP license
• Master's in Nursing or higher
• Valid driver's license + reliable car
• Able to work independently in the field
