Nurse Practitioner - Internal Medicine for Primary Care

Job Summary Tanner Health is seeking a full-time Nurse Practitioner or Physician Assistant to join the Tanner Medical Group Bridge Clinic team which is a transitional care facility designed to “bridge “the gap and support patients during the critical period between hospital discharge and a follow-up visit with a primary care provider (PCP). Job Duties & Responsibilities Provide primary care services including medication management, chronic disease monitoring, coordination of care with specialists and patient education. Job Eligibility Requirements Education & Licensure Requires completion of an accredited Nurse Practitioner program, with a master’s degree in nursing OR master’s degree from an accredited Physician Assistant program Active GA APRN license OR GA Physician Assistant license or meet criteria for obtaining licensure. Must be board certified as a Physician Assistant. Experience Mininum two years' experience required. Prior experience in adult medicine is preferred. Schedule & Location Full-time, Monday – Friday, 8am – 5pm. Position will be located require shifts in Carrollton and Villa Rica Bridge Clinic locations. #J-18808-Ljbffr Tanner-Health-1
