Outpatient Physical Therapist

BAYADA Home Health Care is located in Waynesboro, VA 22980. We are looking for an experienced Physical Therapist, PT for a full‑time, outpatient position serving senior living clients in the Staunton to Charlottesville, VA area. As an Outpatient Physical Therapist, you will be an integral member of a multi‑disciplinary health‑care team that delivers rehabilitative care and skilled nursing to patients, enabling them to remain at home. Responsibilities Make outpatient visits to clients in designated geographic territories. Perform diagnostic tests and measurements, including mobility/range of joint motion, transfer status, stability, ambulation patterns, strength, endurance, balance, and safety assessments. Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions. Continuously assess and revise the PT care plan, and collaborate with nursing in multidisciplinary care planning. Educate and instruct clients, family members, or other representatives on rehabilitative activities necessary to promote health, safety, and independent living. Accurately document observations, interventions, and evaluations using a state‑of‑the‑art touch‑pad tablet. Qualifications A current VA Physical Therapist – PT license. Graduation from an ATSA or AMA approved program, evidenced by transcript or diploma. Ability to work independently and manage time effectively. Strong interpersonal, organizational, and problem‑solving skills. Solid computer skills; prior experience with electronic medical records (EMR) preferred.
