About the Role

We're seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our evening shift team. As an LPN at Uniontown Nursing and Rehab, you'll play a vital role in delivering high-quality care to our residents, providing assistance with daily living activities, administering medications, and collaborating with other healthcare professionals to ensure the best possible outcomes.

Responsibilities

• Provide direct patient care, including administering medications, wound care, and vital sign monitoring

• Assist with daily living activities such as bathing, dressing, and grooming

• Develop and implement comprehensive care plans in collaboration with the interdisciplinary team

• Maintain accurate and detailed records of patient care and treatment

• Participate in quality improvement initiatives to ensure continuous improvement in patient care

Requirements

• Current LPN license in the state of Pennsylvania

• High school diploma or equivalent required

• Previous experience in a long-term care setting preferred
