Social Worker

Schenectady Center seeks a compassionate Social Worker to support residents and families in our skilled nursing and rehab facility. The Social Worker will complete psychosocial assessments, develop care plans, coordinate safe discharges, and provide counseling, crisis intervention, and advocacy. This role collaborates closely with nursing, rehab, and medical teams to promote resident dignity, adjustment, and quality of life. The Social Worker will lead care conferences, connect residents with community resources, maintain regulatory documentation, and contribute to a respectful, resident-centered, team-based culture.
